Statement of Medical Necessity (SMN) Form
The SMN form may be used to gather information needed to communicate with insurance companies to begin the reimbursement process.
Patient Assistance Program (PAP)
Gilead has a Patient Assistance Program that provides Ranexa free of charge to qualified patients residing in the United States.
